Our Mission

To advocate, plan and coordinate services that will allow older adults to remain independent, healthy and productive in their homes and communities.

Case Management Program For The Frail Elderly (CMPFE)

Case Management (also referred top as CMPFE) helps persons recognize and access needed services. It is availablefrail and vulnerable Iowans recognize and access needed services. It is available for persons aged 60 and older.

 

The goal is to prevent or delay institutionalization with home and community based services. Home and Community Based services have proven to be more cost effective than institutional care and to promote a higher quality of life for individuals. The individual and his/her family will make informed choices about services available.

 

Services are tailored to meet each individual's needs. Case managers are able to simplify and streamline the process of defining needs and identifying resources to best meet those needs.

Case Management depends on a multidisciplinary approach to assessment and problem solving; it includes:

  • In-Home comprehensive assessment and evaluation – we will review the physical, functional, social, emotional and medical needs of our consumers.
  • Care planning – Upon completion of assessment, a comprehensive service plan will be created.  We will provide consumers with a list of care recommendations, as well as specific services that will best meet your needs.
  • Coordination of services and appointments – we will arrange an appropriate and cost-effective service from the many health care providers and other social and community resources.
  • On-going monitoring and support/supervision – we will provide ongoing assistance and coordination and keep consumers informed on a regular basis.
  • Reassessment – we will periodical reassess the service needs and evaluate the effectiveness of the services, and recommend adjustments to the service plan.
  • Medicaid applications – we can assist our consumers with the Medicaid application process
  • Information and Referral – we can make referrals to other professionals in the community for any specialized services.
  • Identification of community resources – we can arrange and research accessing free or low cost programs in the community at the most cost effective rates available such as Medicaid Elderly Waiver, Senior Living Trust Fund, Old American’s Act, Medicare, and National Family Caregiver Program.

ELDERLY WAIVER SERVICES
Case management is the portal for accessing the Elderly Waiver Program.
The Elderly Waiver Program is a Medicaid Program for home care options with case management.
Persons who meet all of the following criteria are eligible for elderly waiver services:

  • Age 65 or over
  • In need of nursing home level of care
  • Iowa resident and U.S. citizen or legal foreign person
  • Meet income and resource guidelines
